I am pretty sure this is G. fascicularis which has a more upright habit sprouting from a single base, and tends to have those extra leaves on the flower stems, compared to the similar G. heteromera which is a stoloniferous and spreading plant that sprawls lower to the ground and has shorter flower stems without leaves.
